10/6/2008 LAPSED LICENSES AND CERTIFICATES: Effective September 1, 2008 there are 463 physical therapist licenses and 85 physical therapist assistant certificates that have lapsed. To verify the status of a license or certificate, use the "PT and PTA Search" link above. If there is reason to believe that someone is practicing as a physical therapist, or working as a physical therapist assistant, with a lapsed license or certificate, please contact the Board office. 10/3/2008 NOTICE OF REVISED ADMINISTRATIVE RULES: The Board has revised several rules in Arizona Administrative Code Chapter 24, Article 2 (Licensing Provisions) and in Article 3 (Practice of Physical Therapy). The revised rules include R4-24-302, Use of Titles; R4-24-305, Complaints and Investigations; R4-24-309, Disciplinary Actions; and R4-24-312, Reporting Requirements. Additionally, the Board has adopted a new rule -- R4-24-304 -- that prescribes the requirements for Adequate Patient Records. 11/15/2006 NOTICE OF MEDICAL RECORDS STATUTE AFFECTING ARIZONA LICENSED HEALTH PROFESSIONALS: Effective September 21, 2006, A.R.S. 32-3211 Medical Records; Protocol; Unprofessional Conduct; Corrective action; Exemptions stipulates that all licensed health care professionals, including physical therapists, are subject to certain requirements concerning establishing written protocols for the secure storage, transfer and access of the medical records of the health professional's patients. Failure to comply with the requirements of the statute can result in a finding of unprofessional conduct and a disciplinary order issued by the Board. Arizona licensed physical therapists must indicate compliance with the requirements of the new statute on their application for relicensure in 2008. 5/27/2008 NEW SUBSTANTIVE POLICY STATEMENT - CONTINUING COMPETENCE REQUIREMENTS: On May 22, 2008 the Board adopted a Substantive Policy Statement that clarifies the law with respect to the continuing competence requirements for Arizona licensed physical therapists.
